Health Care Workers Cheated Out of Pay Get Inaccurate W2 Forms

Employees of a home health care agency in Richfield that filed for bankruptcy say they are receiving W2 forms that include wages they never received.

Crystal Care Home Health Agency recently filed for Chapter 7 Bankruptcy protection.

More than 700 Crystal Care workers say the company owes them $1.3 million. Now some of those people say they are receiving the inaccurate W2 forms that include the money they were never paid.

The company says it has reissued correct W2s to the people impacted.

Employees who believe their W2s are inaccurate and did not receive corrected W2s can file Form 4852 with their return.

It's a substitute W2 that allows you to set the record straight.

Crystal Care is expected to stop operations this month. Court records show DHS, which oversees the home health industry, notified them they were ending state funding.

On average Crystal Care received $7-9 million a year.
